This includes, Dementia, Alzheimer’s-Dementia, Parkinson’s Disease, COPD and other Heart Disease, Diabetes, Edema, Strokes, Osteoporosis, Fall Risk, Hip and Knee replacements and the list continues. As many as 20% of our Seniors have 5 or more of these critical conditions. This puts these individuals at a much higher risk for hospitalization, falls, medication error, malnutrition, dehydration and poor health than others.Services
